Please note the enrolment forms below are suitable for all Happy Haven sites. These forms can be filled out and brought into the service, or you may wish to scan and email them if convenient. We are happy to assist families who have difficulty reading or understanding English.
Enrolling begins here! Please take the time to carefully read this document and ask any questions if you are unsure of what is required. One enrolment package PER FAMILY is required. Please note carers and children will each have their own separate CRNs from Centrelink, and each is required to initiate your CCB% and CCR eligibility with us.
You will need one child details form PER CHILD you are wanting to enrol. You will only need to print off this form separately to the enrolment form if you are enrolling more than one child.
All families must fill out a Direct Debit form on enrolment. Debits are processed weekly on Thursdays. Please speak with us to clarify any questions you may have surrounding the direct debit process. One form is required PER FAMILY.
